Job Summary

This position requires a skilled Registered Nurse (RN) to provide high-quality care to terminally ill and palliative patients and their families. The RN, Clinical II, Outpatient will work with a multidisciplinary team to coordinate patient care, manage symptoms, and teach patients and families to maximize quality of life.

Key Responsibilities

* Utilize the nursing process to manage patient care and develop effective care plans.
* Analyze assessment data to determine nursing diagnoses and develop targeted interventions.
* Demonstrate leadership in coordinating patient care across the healthcare continuum, prioritizing essential functions, and utilizing effective critical thinking skills.
* Contribute to a safe and effective working environment, maintaining confidentiality and adhering to patient privacy rights.

Requirements

* 1-2 years of relevant experience as a Registered Nurse in an acute care setting, with a preference for Home Health, Oncology, and/or Hospice experience.
* Effective problem-solving skills, good interpersonal and communication skills, and the ability to relate to a variety of health professionals in different settings.
* EMR experience preferred, along with a current RN license issued by the California Board of Registered Nursing, BSN preferred, and BLS issued by the American Heart Association. Hospice and Palliative Care Certification is also preferred.
