Key Responsibilities

  • Develop or contribute to the development of personalized care plans, identifying providers, specialists, and community resources needed for care.
  • Provide psychosocial and resource support to members, caregivers, and care managers to access local resources or services.
  • Coordinate between members, caregivers, and care providers to ensure timely access to identified care or services.
  • Monitor progress towards care plan goals and collaborate with healthcare providers for plan revision or address identified member needs.
  • Collect, document, and maintain member information and care management activities to ensure compliance with state, federal, and third-party payer regulators.
  • Perform on-site visits to assess member needs and collaborate with providers or resources as needed.
  • Provide education to care managers and members on procedures, healthcare provider instructions, care options, referrals, and healthcare benefits.
Requirements
  • Bachelor's degree and 2-4 years of related experience.
  • Current state's clinical license preferred - LPN.
  • OB experience highly preferred.
