Transportation Coordinator

We are seeking a skilled and compassionate Transportation Coordinator to join our team at BoldAge PACE. As a key member of our Interdisciplinary Team, you will be responsible for the safe and efficient transportation of our frail elderly population to their destinations.
Key Responsibilities- Effectively communicate with participants, families, staff, and other agencies to ensure seamless transportation services.
- Drive a specially equipped van to deliver physically limited persons to varied destinations, ensuring timely arrivals and departures.
- Follow established schedules, adjust schedules as needed, and determine the safest and most efficient pick-up and delivery routes.
- Assist passengers on and off the van in a safe and courteous manner, handling medical equipment and emergencies as needed.
- Inspect the van and maintain it in a clean, orderly, and safe condition, reporting any maintenance needs.
- Participate in staff meetings and take part in participant care planning, communicating any changes in participant condition to team members.
- Observe each participant for any changes in physical, mental, emotional, and social functioning and report such changes to the licensed nurse.
- Complete documentation as required.
- High school diploma or equivalent.
- Minimum 2 years supervisory experience.
- Minimum 1 year experience transporting individuals with special mobility needs.
- 1 year of experience working with a frail or elderly population preferred.
- Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
- Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
- Pass a comprehensive criminal background check.
- Work independently and as an effective member of a team.
- Good understanding of psychosocial problems of aging.
- Social activities, including arts/crafts and performance programming, tailored to senior population.
- Basic computer skills and use of electronic mail.
- Be empathic and patient to attend to needs of frail elderly population.
- Communicate clearly and effectively to participants on a daily basis.
- Proper body mechanics for assisting seniors and persons with mobility limitations.
- Managing special needs populations, including individuals with dementia.
