Job Title: Habilitation Assistant

At Constructive Partnerships Unlimited, we are seeking a dedicated and compassionate Habilitation Assistant to join our team at our Queens Long Island City Day Program.

Job Summary:

The Habilitation Assistant will play a vital role in implementing and monitoring program plans for individuals with intellectual and developmental disabilities. This position requires excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Implement and monitor program plans for individuals with intellectual and developmental disabilities
  • Ensure daily documentation is maintained and use various software for data collection
  • Work directly with individuals and assist with feeding, personal hygiene, and lifting/transferring as needed
  • Ensure the health, safety, and wellbeing of all individuals attending the program
  • Notify management and/or nurse of any medical concerns and accompany individuals to the hospital and/or healthcare provider as needed
Requirements:
  • High School Diploma and/or GED from an accredited school
  • 1 year experience working with individuals with intellectual and/or developmental disabilities
  • Excellent communication and interpersonal skills
  • Ability to work outdoors for community-based activities as needed
  • Ability to perform data entry on a personal computer (PC)
  • Criminal background check
  • Valid NYS Driver's License preferred
  • Will be required to successfully pass a Medication Administration Program
