Job Title: Senior Community Integration Counselor

Description

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are actively seeking Senior Community Integration Counselors in Trevose, PA. This role is responsible for assisting individuals with learning physical, intellectual, emotional, and social life skills.

The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community. The ideal candidate will have a strong background in supporting individuals with intellectual and developmental disabilities and will be able to establish a comfortable and supportive relationship with participants.

Responsibilities:

  • Assist individuals with daily living skills and the development of community involvement, communication skills, problem-solving skills, and social skills
  • Provide ongoing support for community integration efforts
  • Assist with developing a schedule for each participant to maximize community integration
  • Provide training to individuals towards achieving program goals
  • Complete training to facilitate staff development
  • Administer medications and ensure proper completion of necessary documentation
  • Plan, develop, facilitate, and participate in individual and group activities with participants, including leisure and volunteer opportunities
  • Develop individual day program support plans
  • Provide reports on individual progress in relation to overall goals
  • Maintain inventory and ensures adequate supplies for program
  • Oversee consumer funds as needed
  • Monitor the health and medical needs of individuals and immediately report any concerns
  • Drive individuals to medical appointments and activities in the community
  • Ensure program documentation and billable records are completed accurately and timely
  • Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
  • Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation
  • Additional tasks and responsibilities may be assigned

Working Conditions:

  • Transport individuals utilizing your own vehicle or company provided vehicles
  • Schedule may change due to business needs and may include evening and weekend hours
  • May be required to be on-call in cooperation with other management staff
  • Frequent lifting, stretching, and other physical exertion may be required
  • May be required to lift or move 25+ pounds
  • May be exposed to various medical conditions and communicable diseases
