The Transplant Center at Cleveland Clinic Weston Hospital stands out as a premier facility for heart, liver, and kidney transplants in South Florida.

In this role, you will engage closely with leading transplant surgeons and researchers who are pioneering advancements in organ transplantation.

As a Transplant Nurse Coordinator, your primary responsibility will be to oversee pre and post-transplant patient activities, fostering long-term relationships with patients. This position offers the opportunity to make significant impacts in the lives of individuals.


Your work schedule will typically be Monday to Friday, from 8 AM to 5 PM.


Minimum qualifications include:

  • A minimum of two years of RN experience and one year of acute care hospital experience, including disease-related exposure.
  • Basic Life Support (BLS) Certification.

Preferred qualifications for an ideal candidate are:
  • At least two years of RN experience in an ICU or ED setting.
  • A Bachelor's Degree.
  • Strong multitasking and prioritization skills.

Key Responsibilities:
  • Evaluate patients in the outpatient clinic alongside transplant physicians.
  • Facilitate care plans and symptom management with outpatient teams.
  • Coordinate scheduling for follow-up appointments.
  • Review medical information from referring physicians with transplant physicians and initiate necessary communications.
  • Collaborate with various disciplines and gather follow-up information.
  • Work with hospital personnel to ensure proper discharge of transplant patients and donors.
  • Assist in securing medications and supplies based on patient insurance coverage.
  • Reinforce discharge instructions with patients.
  • Communicate with referring physicians to arrange follow-up care.
  • Coordinate evaluations for transplant candidacy with transplant and referring physicians.
  • Manage tissue typing and pre-testing arrangements prior to transplant.
  • Compile evaluation test results and coordinate further testing as needed.
  • Serve as the primary contact for pre-transplant evaluations for physicians and insurance companies.
  • Prepare and present cases to the Selection Committee.
  • Enroll patients on the national transplant waiting list.
  • Oversee ongoing care for inpatients and outpatients awaiting transplants.
  • Assess patients throughout the transplant process for medical and psychosocial concerns, providing timely care as needed.
  • Collaborate with Office Coordinators and Financial Counselors to secure insurance clearances.
  • Organize informational packets for patients prior to transplant.
  • Review transplant information with patients upon arrival and address any questions.
  • Conduct educational sessions for patients, families, and donors before transplant.
  • Work with Organ Procurement Organizations to facilitate organ procurement and transplantation surgeries.
  • Participate in research by implementing protocols and collecting data.
  • Attend relevant conferences and meetings.
  • Facilitate interdepartmental studies and procedures.
  • Provide in-service training for nursing staff and presentations for external organizations.
  • Perform other duties as assigned.

Education:
  • A Bachelor's Degree is preferred.

Certifications:
  • Basic Life Support (BLS) Certification.

  • Must be able to work under stress and during irregular hours.

Personal Protective Equipment:
  • Adhere to Standard Precautions using personal protective equipment as required.
