Job Description

Job Summary:

Community Teamwork Inc is seeking a skilled Youth Housing Case Manager to join our team at the Youth Opportunity Center. As a key member of our Youth Services team, you will be responsible for providing case management services to at-risk and homeless youth between the ages of 16 and 24.

Key Responsibilities:

  • Assessment and Case Planning: Conduct thorough assessments to identify the housing needs of each participant and develop individualized service plans to address their specific needs.
  • Community Outreach and Engagement: Establish and maintain relationships with community partners to provide youth with access to resources and services that promote housing stability and well-being.
  • Advocacy and Support: Provide advocacy and support to youth and families to overcome barriers to self-sufficiency, including housing, education, and employment.
  • Collaboration and Communication: Work closely with other Youth Services team members to ensure a comprehensive and coordinated approach to serving youth and families.

Requirements:

  • Education and Experience: High school diploma or equivalent required; prior related experience and a strong understanding of community resources and services preferred.
  • Skills and Qualifications: Excellent communication and interpersonal skills; ability to work with diverse populations, including at-risk youth and families; proficiency in basic computer software and data collection systems.
  • Preferred Qualifications: Bilingual or multilingual skills; prior experience working with at-risk youth and families; knowledge of community resources and services.
