Are you passionate about helping individuals experiencing homelessness achieve self-sufficiency? As a Case Manager with HELP USA, you will play a vital role in empowering your clients to overcome the challenges of homelessness and build a brighter future.

Key Responsibilities:
  • Conduct thorough assessments to identify the root causes of homelessness and develop personalized independent living plans.
  • Collaborate with a multidisciplinary team to provide comprehensive support services, including employment, education, medical, and mental health services.
  • Establish and maintain regular communication with clients to monitor progress, provide guidance, and facilitate access to necessary resources.
  • Develop and maintain relationships with community partners to ensure seamless referrals and service coordination.
Requirements:
  • Bachelor's degree in social work or a related field, or equivalent experience and skills.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple priorities.
