Job Summary

Community Care, Inc. is seeking a skilled and compassionate Home Health Visit LPN to join our team. As a member of our healthcare team, you will provide direct nursing care to patients in the comfort of their own homes.

Key Responsibilities
  • Implement and evaluate patient care activities under the guidance of a Registered Nurse (RN)
  • Administer prescribed medications and perform specific treatments as ordered by a physician
  • Document patient care activities and treatment responses in a timely and accurate manner
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
  • Participate in ongoing education and training to enhance clinical skills and knowledge
Requirements
  • Graduate of a state-approved school of nursing with a course in pharmacology
  • Current license to practice as a Licensed Practical Nurse (LPN) in the State of Pennsylvania
  • Minimum of six (6) months experience in relevant patient care
  • Current CPR certification
  • Reliable transportation and a valid driver's license
  • Ability to obtain and maintain required clearances, including Childline Clearance, FBI Clearance, and DIG
