Job Title: Physical Therapist - Home Health Care

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Enola, Pennsylvania.

About the Role:

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our adult and geriatric clients in their homes. As a Physical Therapist with BAYADA, you will be an integral part of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive the medical care required to remain at home.

Responsibilities:
  • Perform diagnostic tests and measurements to assess client needs.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
  • Continually assess and revise the Physical Therapy care plan, participating with nursing in the multidisciplinary care plan as needed.
  • Educate and instruct clients, family members, or other client representatives in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Qualifications:
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
