About the Role:We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our adult and geriatric clients in their homes. As a Physical Therapist with BAYADA, you will be part of a multidisciplinary team that provides rehabilitative care and skilled nursing to clients, enabling them to receive the medical care required to remain at home.
Responsibilities:- Perform diagnostic tests and measurements to assess client needs.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assess and revise the care plan, participating with nursing in the multidisciplinary care plan as needed.
- Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
- A current Pennsylvania Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
