About the Role

We are seeking a highly skilled Anticoagulation Clinical Nurse Specialist to join our team at Providence St. Patrick Hospital. As a key member of our healthcare team, you will play a vital role in delivering high-quality, patient-centered care to individuals on anticoagulation medication.

Key Responsibilities
  • Assess patients on anticoagulation medication and make dosage adjustments based on Point of Care Testing (POCT) or laboratory values.
  • Document patient information in Epic and provide detailed education to patients, family members, and healthcare professionals regarding anticoagulation management.
  • Collaborate with referring providers, ACC Pharm-D, front and clinical staff, IHI ACC Clinical Manager, and IHI Practice Manager to ensure efficient and compassionate care.
Requirements
  • Graduate of an accredited RN nursing program.
  • Montana Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association upon hire.
  • 2 years of clinical RN experience, resulting in excellent clinical skills and judgment.
Preferred Qualifications
  • Bachelor's Degree in Nursing.
  • Certification in CACP, CVRN, or Med-Surg upon hire.
  • Recent experience in Anticoagulation Management, Cardiology, or Med/Surg.
  • Decision support experience and Electronic Medical record (EMR) experience.
