Job Title: Physical Therapist Home Health

VNA Health Group is seeking a skilled Physical Therapist to join our Home Health team in Brooklyn Heights, Ohio.

About the Role:

We are looking for a dedicated Physical Therapist to work on weekends, providing high-quality care to patients in their homes. As a Physical Therapist Home Health, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and implementing interventions to improve patient outcomes.

Responsibilities:
  • Provide direct physical therapy services to patients, including performance of physical therapy procedures and instruction of patients and caregivers.
  • Develop and implement treatment plans to improve patient function and mobility.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust treatment plans as needed.
  • Collaborate with other healthcare professionals to ensure comprehensive care.
  • Submit all documentation in a timely manner.
Requirements:
  • Bachelor of Science Degree in Physical Therapy required, post-graduate degree preferred.
  • One year experience in a supervised healthcare setting.
  • Licensed, in good standing, to practice physical therapy in the State of Ohio.
