Physical Therapist Home Health - Weekender

VNA Health Group is seeking a skilled Physical Therapist to join our dynamic team in Brooklyn Heights, Ohio. As a Physical Therapist Home Health - Weekender, you will play a vital role in providing high-quality care to patients in their homes.

Responsibilities:
  • Conduct thorough assessments and evaluations of patient care needs related to functional status, mobility, endurance, and pain management.
  • Develop and implement personalized treatment plans, incorporating physical therapy procedures, patient education, and recommendations for equipment or home environment adaptations.
  • Provide direct physical therapy services, including performance of physical therapy procedures, instruction of patients and caregivers, and facilitation of total care services.
  • Collaborate with healthcare teams to ensure seamless care coordination and patient outcomes.
  • Submit timely and accurate documentation, including patient records and progress reports.
Qualifications:
  • Bachelor of Science Degree in Physical Therapy required; post-graduate degree preferred.
  • One year of experience in a supervised healthcare setting.
  • Licensed to practice physical therapy in the State of Ohio.
  • CPR certification required.
  • Valid Driver's License.
