Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality patient care in a home health setting. As a Physical Therapist with VNA Health Group, you will have the opportunity to work with a dynamic team and make a meaningful difference in the lives of our patients.

Responsibilities:
  • Conduct thorough assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
  • Develop and implement individualized treatment plans with measurable goals and specific discipline-related treatments in conjunction with physical referral/orders.
  • Provide direct physical therapy services, including performance of physical therapy procedures, instruction of patients and/or caregivers, and recommendations for equipment or adaptation of home environments.
  • Re-evaluate and revise treatment plans as needed with physician approval.
  • Facilitate total care services to patients by making appropriate referrals to other disciplines, obtaining physicians' orders, or consulting with case managers.
  • Submit all documents in a timely manner.
Qualifications:
  • Bachelor of Science Degree in Physical Therapy required, post-graduate degree preferred.
  • One year experience in a supervised healthcare setting.
  • Licensed, in good standing, to practice physical therapy in the State of Ohio.
  • CPR certification required.
  • Valid Driver's License.
