Job Summary

  • We are seeking a highly skilled Nurse Care Coordinator to join our team at Staffing Solutions Organization LLC (SSO).

In this role, you will work closely with care coordination capacity in the community to maximize the efficient use of state resources.

The ideal candidate will have 3 years of progressively responsible experience in nursing and/or healthcare services, with a strong focus on patient-centered care.

Key Responsibilities:

  • Develop and implement targeted plans of care to decrease avoidable emergency department usage for MaineCare members.
  • Partner with hospitals to provide ongoing education to MaineCare members and providers.
  • Work with the Care Coordination Unit to monitor work queues and complete assigned tasks.
  • Collect data and prepare records, reports, and statistical information to evaluate wellness programs and the ED Collaborative.

Requirements:

  • Licensed as a Registered Professional Nurse (RN) or Licensed Practical Nurse (LPN) in Maine.
  • Ability to communicate effectively and maintain objectivity in stressful situations.
  • Knowledge of current standards of care and practices related to healthcare facilities, agencies, and practitioners.

Salary and Benefits:

  • Estimated salary: $35.18 per hour.
  • A range of benefits, including hourly pay.
