Responsibilities:




  • Perform patient assessments and develop care plans to meet individual needs
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient vital signs and report any significant changes to the healthcare team
  • Provide education to patients and their families on disease management and self-care
  • Collaborate with the healthcare team to ensure seamless care coordination and communication


Requirements:




  • Active LPN license in good standing
  • CPR certification
  • Previous home health or private duty experience preferred
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
