*The *Clinical Care Coordinator* is tasked with the overall oversight and management of clinical services. This role involves coordinating and supervising a multidisciplinary team to ensure the delivery of high-quality care to home health patients in accordance with the physician's prescribed care plan and all relevant state and federal regulations.

  • Manages referrals and ensures timely assignment of appropriate clinicians and/or therapists for patient evaluations.
  • Coordinates the assessment of patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Continuously assesses patient needs, ensuring that care is tailored to individual requirements, appropriate, and compliant with physician orders.
  • Oversees the development, implementation, and updates of personalized patient care plans as necessary.
  • Documents communications with physicians, clinicians, patients, and referral sources, providing updates to the clinical team as needed.
  • Reviews assessments and care plans daily, consulting with clinicians for recommendations as required.
  • Coordinates care across all disciplines, including physicians, medical equipment providers, family members, and other healthcare professionals.
  • Follows up on laboratory and clinical diagnostic tests, ensuring timely physician notifications and necessary modifications to care plans.
  • Organizes and documents case conferences and reports, facilitating effective communication across disciplines, especially regarding significant findings and patient updates.
  • Receives reports from field clinicians prior to scheduled days off regarding patient status and ongoing needs.
  • Processes orders based on verbal instructions from physicians and communicates these to field staff.
  • Reviews clinician visit documentation weekly to ensure timely and accurate submissions by field personnel. Evaluates service delivery to confirm appropriateness of care and resource utilization through random patient visits and medical record assessments.
  • Enters incidents and occurrences into the online Risk Management Reporting System as per policy.
  • Provides direct patient care as needed, adhering to scope of practice and physician directives.
  • Guides clinicians in implementing best practice interventions when finalizing patient care plans.
  • Participates in on-call rotations as required.
  • Receives reports from weekend and after-hours clinicians regarding new patient admissions.
  • Completes required training courses and additional assignments as requested by management, along with any state-specific training mandated by regulations.
