Position Overview:

The Clinical Liaison plays a pivotal role in the Substance Use Program, reporting directly to the Program Manager. This position is essential in enhancing the clinical operations of the substance use team.

As the primary contact for both patients and clinicians, the Clinical Liaison ensures that individuals enrolled in the program receive comprehensive care. Collaboration with the nursing staff is crucial for effective service delivery.


Key Responsibilities:

Patient Engagement and Support:
1. Facilitate the intake process for individuals seeking assistance through internal or external referrals.
2. Provide support to patients navigating services such as detoxification or further treatment options.
3. Ensure timely access to medications for substance use disorder treatment through coordination of refills and authorizations.
4. Collaborate with nursing staff during care delivery to optimize patient flow.
5. Establish a compassionate rapport with patients to foster effective engagement.
6. Organize group sessions as directed by clinical professionals.

Administrative Duties:
1. Maintain accurate and confidential patient records in accordance with established guidelines.
2. Keep updated patient lists to facilitate care coordination.
3. Assist in scheduling appointments and follow-ups to ensure seamless patient care.
4. Work alongside the administrative team to enhance operational efficiency and communication.

Resource Coordination:
1. Help patients access community resources such as housing assistance and job placement services.
2. Collaborate with external organizations to facilitate referrals and support networks.

Team Collaboration:
1. Serve as a liaison among clinical staff, administrative personnel, and patients to promote effective communication.
2. Participate in team meetings to share insights on patient progress and needs.

Compliance and Ethical Standards:
1. Adhere to confidentiality protocols and ethical standards in managing patient information.
2. Stay informed about relevant policies and regulations regarding substance use treatment and healthcare practices.

Qualifications:
Skills and Experience:
- Preferred experience as a Medical Assistant or willingness to acquire clinical support skills.
- Background in direct service organizations, particularly in collaboration with clinical teams.
- Highly organized and self-motivated individual capable of managing workloads independently.
- Ability to practice trauma-informed care with individuals experiencing substance use disorders.
- Enjoyment in facing challenges and working within a diverse environment.
- Experience in data management and analysis, including patient tracking using digital tools.
- Familiarity with Electronic Medical Records.
- Commitment to enhancing substance user health and addressing social determinants of health.
