Job Description:Key Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans for patients receiving home health services.
- Provide skilled nursing care, including wound care, medication management, and patient education, to promote optimal health and independence.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate holistic care and support services for patients.
- Monitor patient progress, evaluate outcomes, and adjust care plans as needed to ensure continuity and quality of care.

Qualifications:
