Job Summary: The Intake Coordinator plays a vital role in ensuring seamless patient transitions to home care. This position is responsible for managing all aspects of the patient intake process, including communication with patients and families, data entry, and establishing positive relationships with referral sources. The ideal candidate will have healthcare experience, preferably in referrals/intake in a home health or hospice environment, and possess excellent communication, negotiation, and public relations skills.
Key Responsibilities:
- Directs daily patient referral and intake operations, providing assistance with oversight of intake policies and ensuring compliance with regulatory requirements.
- Provides insight and assistance with implementing improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Establishes and maintains positive working relationships with current and potential referral sources.
- Ensures seamless transition of patients to home care by assisting with oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
- Ensures maximum third-party reimbursement through direct oversight of insurance verification and authorization processes.
Requirements:
- Healthcare experience, preferably in referrals/intake in a home health or hospice environment.
- Excellent communication, negotiation, and public relations skills.
- Autonomy, assertiveness, and cooperation in performing job responsibilities.
