Hospice Nurse Job Description

We are seeking a compassionate and skilled Hospice Nurse to join our team at Custom Care Hospice. As a Hospice Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families in the Southern End of Dallas County.

Key Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide palliative nursing care, including pain and symptom management, to improve patients' quality of life.
  • Collaborate with the interdisciplinary team to coordinate patient care and ensure seamless transitions between care settings.
  • Communicate effectively with patients, families, and healthcare professionals to ensure comprehensive care.
  • Supervise and delegate tasks to hospice aides as needed.
Requirements:
  • Graduate of a National League for Nursing accredited school of nursing.
  • Minimum of two years of experience in public health, home care, or hospice nursing.
  • Current licensure to practice professional nursing in the state.
  • Excellent observation, problem-solving, verbal, and written communication skills.
  • Ability to organize and prioritize workload independently.
