Job Summary

This MDS Care Coordinator plays a critical role in conducting and coordinating resident assessments in accordance with state requirements and facility policies. Key responsibilities include developing and completing preliminary and comprehensive resident assessments (MDS) and ensuring comprehensive care plans meet residents' medical, nursing, mental, and psychosocial needs.

Key Responsibilities
  • Conducts and Coordinates Resident Assessments: Develops and completes preliminary and comprehensive resident assessments (MDS) in accordance with current rules, regulations, and guidelines.
  • Ensures Comprehensive Care Plans: Ensures care plans include measurable objectives and timetables to meet residents' medical, nursing, mental, and psychosocial needs.
  • Completes Initial Resident Assessments: Completes initial resident assessments within 14 days of admission and ensures each portion is signed and dated by relevant staff members.
  • Interviews Residents: Conducts or coordinates interviews with residents for assessment purposes.
  • Develops and Updates Care Plans: Helps develop and update comprehensive resident assessments and care plans, ensuring quarterly and annual updates, and revisions upon significant changes in condition.
Requirements
  • Experience: 2-4 years' experience in direct care provision to multiple patients/residents, preferably in a Skilled Nursing environment; previous MDS experience required.
  • Education: Nursing degree from an accredited college or graduation from an approved LPN/RN program.
  • Licenses and Certifications: Current, unencumbered LPN license in Colorado; RN license preferred; current valid CPR certification.
Benefits and Compensation

The estimated annual salary for this position is $65,000 - $85,000, including benefits such as medical insurance, dental insurance, vision care, and 401k plan. Paid vacation time also applies.
