Job Summary

This position is a critical part of the Fresenius Medical Care team, providing essential care to patients with end-stage renal disease. As a Home Therapies RN CAP 1, you will work closely with patients and their families to ensure they receive the best possible care.

About the Role
  • Provide comprehensive care to patients undergoing home dialysis treatments
  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Conduct assessments and evaluations to identify patient needs and adjust treatment plans accordingly
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
  • Participate in quality improvement initiatives to enhance patient outcomes and facility operations
Responsibilities
  1. Perform ongoing collection and analysis of dialysis data for assigned patients
  2. Develop and implement patient education programs to promote self-care and independence
  3. Assess and address barriers to effective treatment, including environmental and logistical challenges
  4. Lead and mentor other staff members to ensure high-quality patient care
  5. Participate in on-call rotation to provide 24/7 coverage and support
Requirements
  • Graduate of an accredited School of Nursing
  • Current appropriate state licensure
  • CPR BLS Certification
  • Minimum 12 months experience as a RN (preferred)
  • Home dialysis therapy experience (preferred)
