Position: Care Management Coordinator

The Care Management Coordinator plays a vital role in formulating and executing care strategies for patients with intricate needs to maximize their healthcare outcomes. This position entails managing patient health records, facilitating care transitions, evaluating patient requirements, delivering education, and ensuring access to necessary resources.

Key Responsibilities
  • Document patient health data in the electronic medical record (EMR) and uphold care plans
  • Oversee transition of care services following hospital discharge
  • Identify high-risk patient groups and create tailored care management strategies
  • Conduct evaluations and provide educational support to patients
  • Engage in Interdisciplinary Care Team discussions
  • Support utilization management for high-risk populations
  • Execute initiatives aimed at promoting cost efficiency
Qualifications
  • High school diploma or equivalent
  • A minimum of two years of relevant experience is preferred
  • Certification as a Medical Assistant, LPN, Health Coach, or CNA is required
