About the Position

We are seeking a dedicated Social Worker to join our team at NewCourtland Senior Services in Philadelphia, PA.

Job Summary:

  • Conduct comprehensive assessments to determine clients' needs and develop individualized care plans.
  • Provide counseling and support services to promote recovery and stability.

Responsibilities:

  • Collaborate with a multidisciplinary team to deliver comprehensive care services.
  • Facilitate access to community resources, including housing, medical care, employment services, and educational opportunities.
  • Maintain accurate records of client progress and services provided.

Requirements:

  • Bachelor's degree in social work from an accredited institution (preferred).
  • Minimum 3 years of experience working with adults in substance abuse, homelessness, and behavioral health settings.
  • Exceptional communication and interpersonal skills.
