Job Title: Hospice Registered Nurse

At Liberty Hospice, we are seeking an experienced Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will play a vital role in providing high-quality care to our patients and their families.

Job Summary:

We are looking for a compassionate and skilled Registered Nurse to provide end-of-life care to our patients. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a team environment.

Responsibilities:
  • Provide direct patient care, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and coordination of services.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Current RN licensure in the state of practice.
  • Minimum of 2 years of clinical nursing experience, with at least 1 year of hospice nursing experience preferred.
  • Knowledge of nursing theories and concepts, as well as reimbursement guidelines.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
