About the Role

We are looking for a dedicated Client Advocate Specialist to join our team at Greenberg-Larraby, Inc. (GLI).

Job Summary

The Client Advocate Specialist will play a critical role in coordinating care and ensuring that clients receive the necessary services and support to meet their health and wellness goals.

Responsibilities:
  • Assess client needs and develop individualized care plans in collaboration with clients and their support systems.
  • Coordinate services and resources, ensuring accessibility and continuity of care.
  • Maintain accurate and up-to-date client records in compliance with confidentiality and documentation standards.
  • Advocate for clients to access necessary services and support networks.
  • Conduct follow-ups and reassessments to evaluate progress and adjust care plans as needed.
Requirements and Qualifications

To succeed in this role, you will need:

  • A Bachelor's degree in social work, nursing, psychology, or a related field.
  • A minimum of 2 years of experience in case management or a related field.
  • Knowledge of community resources and services.
  • Strong organizational skills and the ability to handle multiple priorities.
  • Excellent written and verbal communication skills.
  • Ability to work independently and as part of a team.
Compensation and Benefits

This role offers a competitive salary of $60,000 - $80,000 per year, depending on experience, as well as a comprehensive benefits package, including:
