Job Title: Case Manager

We are seeking a skilled Case Manager to join our team at Mass., Inc. The successful candidate will provide case management services for eligible youth under the age of 22, with a focus on those below the age of 18.

Job Summary:

The Case Manager will work closely with families, guardians, and other community providers to ensure integrated planning and service provision. This includes providing creative engagement strategies, regularly scheduled face-to-face contact, and monitoring safety in the client's environment.

Key Responsibilities:
  • Provide case management services for eligible youth under the age of 22, with a focus on those below the age of 18.
  • Engage families and guardians in the process to align with CYF Family Collaboration Principles.
  • Develop and implement individualized service plans to meet the unique needs of each client.
  • Collaborate with internal and external providers to ensure seamless service delivery.
  • Prepare and maintain accurate documentation, including progress notes and other required records.
Requirements:
  • At least three years of full-time or equivalent part-time experience in human services work or social work.
  • Knowledge of psychosocial problems, types, and symptoms of mental and emotional disorders.
  • Ability to communicate effectively orally and in writing, with a focus on clear and accurate instructions.
  • Ability to make decisions quickly and maintain a calm manner in stressful or emergency situations.
Preferred Qualifications:
  • Three years' experience in child/adolescent services.
  • Multilingual fluency.
