Job Title: Case Manager

The Salvation Army Central Territory is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive case management services to clients in our Rapid Rehousing program.

Key Responsibilities:
  • Provide strength-based, client-centered case management to Rapid Rehousing clients.
  • Maintain accurate and up-to-date client information in casefiles and through database entry.
  • Coordinate recruitment, referrals, screenings, and intakes for the Rapid Rehousing program.
  • Engage in internal and external relationship building to better serve clients, develop new resources, and achieve program goals.
  • Maintain a current catalogue of local community housing resources and providers.
Requirements:
  • Bachelor's degree in a relevant field is required.
  • Previous social services experience is necessary.
  • Experience conducting case management is required.
  • Must pass background checks, possess a valid driver's license, and meet MVR requirements.
Competencies:
  • Flexible interpersonal and communication skills.
  • Excellent leadership and organizational skills and attention to detail.
  • High level of honor in dealing with confidential information.
  • Works with integrity, valuing honesty and diligence.
  • Knowledge of social services, community resources, and harm reduction/housing first models.
  • Proficiency with PC, Microsoft Office, databases, and standard office equipment.
Working Conditions:
  • Mostly sedentary work, sitting for long periods of time and working on a computer.
  • Professional environment; eligible for a remote-friendly work environment.
  • Must be able to communicate clearly in English, and work in person or on the phone.
  • Must be able to occasionally lift 30 pounds.
  • Standard business hours with occasional evenings and weekends.
Supervisory Responsibilities:

None.

Travel Requirements:

Will require frequent travel in Northeast Minnesota 6 county area and rare out-of-area travel.
