Job Summary

We are seeking a skilled Case Manager to join our team at the Department of Mental Health. As a Case Manager, you will be responsible for providing case management services to eligible youth under the age of 22, with a focus on those under 18. You will work closely with families and guardians to align with our Family Collaboration Principles and provide creative engagement strategies to engage and sustain involvement in attaining individual goals and maintaining community tenure.

Key Responsibilities
  • Provide case management services for eligible youth under the age of 22, with a focus on those under 18.
  • Engage families and guardians in the process to align with our Family Collaboration Principles.
  • Develop and implement creative engagement strategies to engage and sustain involvement in attaining individual goals and maintaining community tenure.
  • Work with a team of internal and external providers and family to ensure integrated planning and service provision.
  • Prepare and maintain documentation according to DMH regulations and Electronic Health Record (EHR) business rules.
  • Provide regularly scheduled visits to clients' homes or usual places of residence, as appropriate, and monitor safety of the environment.
  • Coordinate and interact with multi-service/system team (family, team, school, other state and community systems) to ensure effective service delivery.
Requirements
  • Knowledge of psychosocial problems, types and symptoms of those with mental and emotional disorders.
  • Knowledge of the principles, practices and techniques of counseling, interviewing, crisis intervention, motivation and reinforcement.
  • Knowledge of the types of programs and services available to the mentally ill population.
  • Ability to understand, explain and apply the laws, rules, regulations, policies, procedures, specifications, standards and guidelines governing agency and Site activities.
  • Ability to make case assessments by examining records and documents and through questioning and observing clients.
  • Ability to communicate effectively orally and in writing, giving clear accurate instructions.
  • Ability to make decisions, act quickly and maintain a calm manner in stressful or emergency situations.
Preferred Qualifications
  • 3 years' experience in child/adolescent services.
  • Multilingual fluency.
Working Conditions

This position requires travel to various locations throughout the DMH Southeast Area and occasional statewide travel to acute inpatient hospitals or other treatment settings. A current and valid Massachusetts Class D (or equivalent from another state) Motor Vehicle Operators license is required.
