Job Summary

The RN Case Manager is responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence. This role requires a strong understanding of community health nursing therapy and the ability to work independently or as part of an interdisciplinary team.

Key Responsibilities
  • Provide evaluation and nursing services to patients in their homes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Conduct regular assessments and evaluations to monitor patient progress
  • Document all patient interactions and care provided in the patient's medical record
Requirements
  • Graduate of an accredited school of nursing
  • Current Washington State Registered Nurse License
  • National Provider BLS - American Heart Association certification
  • National Provider Identifier (NPI) number
Preferred Qualifications
  • Bachelor's Degree in Nursing
  • 2 years of full-time experience practicing as a registered nurse in a Long Term Care, Medical/Surgical Unit, and/or Skilled Nursing Facility
  • 1 year of recent, hands-on technical clinical experience in a Medicare-certified Homecare or Hospice agency/facility doing intermittent visits and case management
  • Recent hands-on experience in wound care, venipuncture, ostomy, catheters, and feeding tubes
  • Rehab nursing experience
  • Experience working with an EMR system
  • Experience with office software programs including Outlook, Word, and/or Excel
