Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team at Providence Home and Community Care. As a RN Case Manager, you will be responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence.

Key Responsibilities
  • Provide evaluation and nursing services to patients in their homes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Conduct regular assessments and evaluations to monitor patient progress
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
Requirements
  • Graduate of an accredited school of nursing
  • Current Washington State Registered Nurse License
  • National Provider BLS - American Heart Association certification
  • National Provider Identifier (NPI) number
Preferred Qualifications
  • Bachelor's Degree in Nursing
  • 2 years of full-time experience practicing as a registered nurse in a Long Term Care, Medical/Surgical Unit, and/or Skilled Nursing Facility
  • 1 year of recent, hands-on technical clinical experience in a Medicare-certified Homecare or Hospice agency/facility doing intermittent visits and case management
  • Recent hands-on experience in wound care, venipuncture, ostomy, catheters, and feeding tubes
  • Rehab nursing experience
  • Experience working with an EMR system
  • Experience with office software programs including Outlook, Word, and/or Excel
