Join CenterWell Home Health as a skilled Licensed Practical Nurse for Home Health. As a member of our team, you will provide professional nursing services under the direction of an RN. Your responsibilities will include providing skilled nursing interventions for patient/client illness, rehabilitative needs, and preventative care. You will apply your knowledge and skills to facilitate problem resolution and achieve individualized patient goals and outcomes. You will also maintain contact with patients, physicians, clinical managers, and other healthcare team members. Additionally, you will maintain the highest standards of professional conduct in relation to confidential information and prepare, submit, and maintain required documentation. To be successful in this role, you will need to have a graduate degree from an accredited Licensed Practical Nursing Program, a current nursing license in the practicing state, a valid driver's license, auto insurance, and reliable transportation. You will also need to have current CPR certification and at least two years of experience as an LPN/LVN in a clinical setting.
