We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Signature Healthcare at Home. As a Home Health LPN, you will provide direct patient care under the supervision of a registered nurse, following a comprehensive assessment and plan of care.
Responsibilities- Provide direct patient care as defined in the State Nurse Practice Act.
- Implement current nursing practice, following a comprehensive assessment and the plan of care.
- Document patient services accurately and timely, reflecting the plan of care.
- Evaluate patient's response to treatments/medications and provide education to patients and families/caregivers.
- Use and prepare equipment and supplies effectively and efficiently.
- Participate in personal, professional growth and development, and organization's quality assessment performance improvement teams and activities.
- Graduate of an accredited practical nurse or vocational nursing program.
- One (1) to two (2) years nursing experience preferred, with community health or medical/surgical experience.
- Flexible work schedules: PRN, Part-Time or Full-Time, 10-hour shifts available, 30-to-40-hour work week, benefits offered for Part-Time and Full-Time employment.
