Clinical Care Specialist

The Clinical Care Specialist will provide exceptional medical care to patients by conducting physical exams, ordering and interpreting diagnostic tests, and developing patient treatment plans under the supervision of a physician. This role is a safety-sensitive position as it involves direct patient care and administering medication.
Key Responsibilities:
• Practice medicine under the supervision of a physician using sound medical judgment and adhering to all ethical considerations of the practice of medicine;
• Provide care for patients with acute and chronic illness encountered in the older adult patient;
• Assess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence-based medicine and practice guidelines;
• Follow all internal policies and procedures;
• Maintain complete and detailed records, electronically in the EMR, of patients health care plans and prognosis, completed within 48 hours of each patients visit;
Requirements:
• Graduate of an accredited school of nursing;
• Certified as a nurse practitioner with current state licensure;
• At least 1 year of previous professional nursing experience with 1-3 years experience as a practicing as a primary care nurse practitioner;
• Prior experience working in an electronic medical records system;
• Provide 5 Star service in accordance with the Better Health Group's mission;
• Skilled in traditional nursing disciplines of patient assessment and management;
• ACLS/BLS certification required as mandated by state;
• Must demonstrate knowledge of the rationale of appropriate patient care;
• Demonstrate ability to handle emergency situations calmly and effectively;
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations;
• Ability to effectively present information and respond to questions from a various group of people;
• Ability to define problems, collect data, establish facts, and draw valid conclusions;
• Excellent written and verbal communication, teaching ability, and good telephone skills;
• Working knowledge with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration;
Physical Requirements:
• Full range of body motion, including handling and transferring patients, manual and finger dexterity; including eye-hand coordination;
• Requires standing, walking, pushing, bending, kneeling, and reaching at arms length and overhead in a clinic for prolonged periods of time;
• Must be able to lift and move patients and medical equipment as needed;
• Requires ability to lift/carry up to 15 lbs;
• Requires corrected vision and hearing to normal range;
• Ability to sit for extended periods of time;
• Ability to operate a motor vehicle and have own means of transportation;
