About the Position:
The Case Manager I role at WellSpace Health offers a unique opportunity for a dedicated professional to make a meaningful difference in the lives of patients. Key Responsibilities:
- Conduct individualized assessments to understand patient needs and develop tailored support plans
- Collaborate with patients to set and achieve goals, promoting empowerment and self-sufficiency
- Provide advocacy and support in accessing necessary resources, including medical care, housing, and employment assistance
- Participate in care coordination meetings with healthcare providers and community organizations
- Develop and maintain relationships with community partners to ensure seamless referrals and support

Requirements:
- High school diploma or equivalent, with preference for Associate Degree in social services or related field
- Strong communication and interpersonal skills, with experience working with diverse populations
- Ability to maintain confidentiality and adhere to HIPAA guidelines
- Proficiency in Microsoft Office applications, including Word, Excel, and Outlook
- Valid driver's license with insurable driving record

Estimated Salary Range:$31.37 - $40.45 per hour, depending on experience.
