Key Responsibilities:
  • Facilitate home health referrals and ensure seamless coordination with patients, families, and healthcare teams.
  • Collaborate with home care staff to provide accurate information on home care plans, order necessary equipment, and communicate with the home health agency.
  • Obtain physician orders for timely initiation of care and document interactions with patients and healthcare providers.
  • Develop strong relationships with referral sources, patients, and their families to promote effective care coordination.
