The Case Management Social Worker will work closely with patients, families, and healthcare providers to coordinate care and services, address psychosocial needs, and facilitate timely discharges. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Conduct thorough psychosocial assessments and develop individualized care plans
  • Coordinate with community resources and supportive counseling services to address patient needs
  • Provide crisis interventions and facilitate transitions of care for high-risk patients
  • Manage all aspects of discharge planning to ensure timely and safe discharges
  • Communicate effectively with interdisciplinary teams and educate patients, families, and physicians on post-acute options
  • Advocate for patients and families, addressing social, psychological, financial, and behavioral barriers to successful medical outcomes
Requirements:
  • Master's degree in social work (MSW) or Master of Science in Social Administration (MSS)
  • Current state licensure as a Social Worker (LSW, LISW or LISW-S)
  • Basic Life Support (BLS) certification through the American Heart Association (AHA)
  • Flexible, open-minded, and adaptable to change
Preferred Qualifications:
  • Certification as a Case Manager within one year of eligibility from an approved professional organization
  • Three years of experience in a healthcare system, including exposure to and experience in pre- and post-acute care
  • Hospital social work experience and knowledge of community services
