Job Summary

We are seeking an experienced Rehabilitation Program Manager to join our team at HealthPRO Heritage in Salem, OR. As a key member of our rehabilitation department, you will be responsible for managing and administering all aspects of rehabilitation services for our communities.

About the Role

The Rehabilitation Program Manager will oversee the delivery of high-quality rehabilitation services, ensuring that our residents receive the best possible care. This role requires strong clinical knowledge, excellent leadership skills, and the ability to work effectively with interdisciplinary teams.

Responsibilities

  • Maintains a clinical caseload and demonstrates strong clinical knowledge and understanding of rehabilitation principles.
  • Understands federal, state, and local laws, rules, and regulations related to therapy service delivery and ensures staff compliance.
  • Demonstrates good planning, organizational, and interpersonal skills necessary to maintain a safe and goal-directed work environment.
  • Represents HealthPRO-Heritage professionally and represents the company as the point of contact for community customer concerns or issues.
  • Serves as a liaison between management, home health, hospice, and community administration and communicates assertively, diplomatically, and clearly through informal and formal mechanisms.
  • Integrates clinical programs and addresses community-specific needs related to marketing.
  • Participates in quality improvement programs and patient care meetings while adhering to HIPAA guidelines.
  • Provides education to the community, physicians, and other healthcare providers regarding therapy and facility services.
  • Plans, schedules, coordinates, and directs rehabilitation services, including approval of vacation time.
  • Determines staffing needs and schedules to meet resident care demands and assists with recruiting interviews as needed.
  • Promotes positive employee relations by facilitating open communication and problem-solving through scheduled team meetings.
  • Conducts performance appraisals of rehabilitation personnel and collaborates with supervisory personnel to review performance and establish goals.
  • Monitors and assists with personnel compliance standards, including required credentials, corrective actions, and improvement plans when needed.
  • Investigates, follows up, and assists in resolving staff, service, and resident-related issues as requested.
  • Develops special programs for resident participation, such as fitness, dementia programming, and various protocols for activities that strengthen and enhance the existing therapy program.
  • Provides leadership to and directly manages clinical therapy staff, facilitating a team atmosphere among staff.

Requirements

  • Bachelor's degree in Occupational Therapy or Physical Therapy or a Master's degree in a related field.
  • Valid state professional licensure.
  • Ongoing continuing education and professional development.
  • Ability to maintain confidentiality and handle sensitive information.
  • Excellent communication, leadership, and problem-solving skills.
  • Ability to work independently with periodic support and direction.
