Position Overview:

We are seeking a dedicated Home Health Aide to provide essential care in the comfort of patients' homes. This role is vital in ensuring the well-being of individuals under the guidance of a registered nurse or qualified therapist.

Key Responsibilities:

  • Deliver compassionate home health aide services to patients in their residences.
  • Exhibit strong communication skills, enabling effective reporting of clinical information to patients, families, and healthcare team members.
  • Monitor and document patient conditions and the care provided, ensuring accurate records of vital signs such as temperature, pulse, and respiration.
  • Implement standard safety precautions and infection control measures diligently.
  • Timely report any changes in patient health status to the nursing supervisor.
  • Maintain a safe, clean, and healthy environment for both patients and colleagues.
  • Recognize emergencies and execute appropriate emergency protocols.
  • Understand and respect the holistic needs of patients, prioritizing their privacy and confidentiality.
  • Utilize safe techniques for personal care, including assistance with mobility and daily activities.
  • Complete all necessary documentation promptly and accurately.
  • Deliver quality care in accordance with the Aide Service Plan and contribute to its ongoing development.
  • Fulfill required training hours as per agency standards.
  • Seek guidance when necessary and adhere to agency policies and procedures.
  • Engage in interdisciplinary evaluations to enhance patient care and contribute to care plan adjustments.
  • Participate in quality assurance and performance improvement initiatives.
  • Assume additional responsibilities as needed.
