Position Summary:

The Home Health Aide is tasked with delivering direct, compassionate care to clients in their homes, under the guidance of a registered nurse. This role involves adhering to the established care plan, executing treatments, and collaborating with the healthcare team to achieve optimal client outcomes.

  1. Deliver direct patient care as outlined by state regulations
  2. Implement care plans and treatment protocols
  3. Maintain accurate and timely documentation
  4. Conduct ongoing assessments of client needs
  5. Provide counseling, education, and emotional support
  6. Monitor and report changes in client condition
  7. Assist with discharge or transfer processes
  8. Educate clients and their families/caregivers
  9. Administer medications and document accordingly
  10. Initiate emergency procedures when necessary
  11. Coordinate home health services effectively
  12. Perform wound care and demonstrate infection control

  14. Utilize equipment safely and effectively

Qualifications:

  1. Graduated from an accredited nursing program
  2. Possess an unencumbered Practical or Vocational Nurse license
  3. Minimum of one year nursing experience required; three years preferred
  4. Adhere to professional standards of practice
  5. Exhibit strong interpersonal skills
  6. Certified in CPR
  7. Demonstrate strong clinical abilities
  8. Provide excellent customer service
  9. Show empathy towards client needs
  10. Work effectively within a team
  11. Self-motivated with minimal supervision
  12. Valid driver's license and insured vehicle
  13. Negative TB test or chest x-ray

Work Environment:

Interactions with clients occur in various settings, which may include exposure to infectious materials. Personal protective equipment may include gloves, masks, and eye protection. Physical demands include the ability to lift up to 50 pounds and perform various physical tasks.
