Nurse Practitioner

Brighton Hospice is seeking a compassionate and skilled Nurse Practitioner to join our interdisciplinary team. As a key member of our care team, you will provide comprehensive care to hospice patients and their families, working collaboratively with our clinical team to deliver exceptional patient outcomes.
Responsibilities- Provide oversight of clinical and Nurse Practitioner services in coordination with the Medical Director
- Assist with on-call needs when the Medical Director is unavailable
- Conduct initial and ongoing assessments of patient needs, developing individualized plans of care to address physical, functional, psycho-social, and environmental needs
- Partner with the interdisciplinary team to implement and manage care plans, determining scope, priority, and frequency of services based on patient needs
- Perform additional duties as needed to ensure high-quality patient care

We are seeking a Nurse Practitioner with:
- At least 1 year of experience in hospice care
- An active Advanced Practice Registered Nurse (APRN) license
- Ability to travel within the service area of Brighton
- Compassion and empathy in caring for patients and their families
