About the Role

EverHeart Hospice is seeking a highly skilled and compassionate Nurse Practitioner to join our team. This full-time position involves working Monday to Friday from 8:00 am to 4:30 pm, with weekend, holiday, and on-call responsibilities.

The successful candidate will support the Medical Director in overseeing medical care for hospice and palliative care patients. Key responsibilities include offering guidance to healthcare professionals and team members to uphold high-quality patient care standards.

This role requires clinical leadership and proficiency, as well as the ability to act as a liaison between programs, physicians, and other healthcare professionals to enhance patient care services.

Key Responsibilities
  • Routine Homecare: Oversee implementation of patient plans, evaluate and modify as necessary to meet patient needs, and consult with the Medical Director when necessary.
  • Prescribe medications to the extent delegated and licensed after collaboration with the Medical Director.
  • Collaborate with interdisciplinary teams concerning admissions and referrals.

Palliative Care: Collaborate with attending physicians to help manage disease progression, perform symptom management, educate patients regarding disease processes, coordinate providers, and link patients to available resources to enhance quality of life.

Requirements
  • Master's Degree in Nursing (MSN) from an accredited School of Nursing.
  • Current Nurse Practitioner license in the State of Ohio.
  • Specialization in area of Family Practice, Oncology, or Hospice is preferred.
  • Minimum of five years of nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care.
Salary and Benefits

We offer a competitive salary of $85,000 - $110,000 per annum, depending on experience, along with a comprehensive benefits package that includes medical, dental/vision benefits, PTO, sick time/paid holidays, 401(k), and tuition reimbursement.
