About This Role
As a dedicated Palliative Care Nurse Practitioner, you will be part of our team that reimagines care for patients and their families.
Responsibilities:
- Deliver high-quality patient care, prioritizing empathy and compassion.
- Collaborate with multidisciplinary teams to provide seamless care transitions.
- Evaluate patient needs, develop personalized care plans, and coordinate services.
Your Qualifications
To excel in this role, you should possess:
- A graduate degree in nursing from an accredited institution.
- At least one year of experience as a Nurse Practitioner, preferably in palliative care.
Certifications and Licenses:
- A valid DEA license in the state where care is being provided.
- A current CPR certification, preferred.
- A driver's license and ability to travel to all business locations.
