Palliative Care Nurse Practitioner Specialist

We are seeking a compassionate and empathetic Palliative Care Nurse Practitioner to join our team at Constellation Health Services.
About the Role:
- This role involves providing high-quality patient care in various settings, including patient homes and facilities.
- The successful candidate will perform comprehensive assessments, coordinate care plans, and communicate with patients, families, and healthcare teams.
- Responsibilities include administering medications, reporting on patient status, and maintaining accurate documentation.
- Participation in interdisciplinary meetings and collaboration with institutions to ensure continuity of care is essential.
Requirements:
- A Master's degree or higher in nursing from an accredited institution is required.
- National Board Certification as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP) is mandatory.
- Experience in hospice or palliative care, clinical oncology, geriatrics, or community health is preferred.
- Palliative care certification (NBCHPN) or hospice certification (ACHPN) is highly valued.
