The Role of Home Health Occupational Therapist

As a Home Health Occupational Therapist, you'll play a vital role in helping our patients regain independence and achieve their full potential. With your expertise and compassion, you'll work collaboratively with our multidisciplinary team to develop personalized care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Conduct thorough assessments and develop comprehensive care plans for patients requiring occupational therapy services;">
  • Implement and adjust treatment plans based on patient progress and changing needs;">
  • Collaborate with healthcare teams to ensure seamless care transitions and optimize patient outcomes;">
  • Communicate effectively with patients, families, and caregivers to promote understanding and engagement in the care process;">
  • Maintain accurate records and documentation, adhering to regulatory requirements and company standards;">

Requirements and Qualifications

To be successful in this role, you'll need:

  • A Master's degree in Occupational Therapy or equivalent;">
  • Licensure or certification in the state where services will be provided;">
  • At least two years of experience working independently under minimal supervision;">
  • Ability to travel to all business locations and possess a valid driver's license;">
