Position Overview:

This is a Full-Time Day Shift role, typically from 8:00 AM to 4:30 PM, Monday through Friday, with rotating weekends. As a Clinical Nurse Case Manager, you will elevate patient care by delivering clinical expertise and compassionate support.

Key Responsibilities:

  • Promote patient-centered care by coordinating comprehensive care plans.
  • Manage patient length of stay and resource utilization effectively.
  • Develop safe and appropriate discharge plans in collaboration with the healthcare team.
  • Conduct thorough assessments of patients' psychosocial, medical, and discharge needs.
  • Identify and mitigate readmission risks through education and resource access.
  • Facilitate effective communication among healthcare team members, patients, and families.
  • Document care coordination activities and recommendations accurately.
  • Ensure compliance with regulatory requirements and organizational policies.

Qualifications:

To be considered for this role, candidates must be graduates of an accredited nursing program, with an Associate's Degree required and a Bachelor's Degree preferred. A minimum of 3 years of clinical hospital nursing experience is essential, with 2 years of critical care experience preferred.
