We are seeking a dedicated and compassionate QMHP Case Manager to join our team at the Child and Family Guidance Center. As a key member of our mental health services team, you will play a pivotal role in providing family-oriented, individual, and group rehabilitative services to children and families in need.
Key Responsibilities:- Provide curriculum-based rehabilitative services to families, individuals, and groups, ensuring compliance with all relevant laws, regulations, and confidentiality requirements.
- Maintain accurate and up-to-date case records, including notes, treatment plans, and uniform assessments, while maintaining professional certification/licensure.
- Collaborate with internal and external stakeholders to coordinate care and referrals, ensuring seamless service delivery.
- Bachelor's degree in rehab counseling, psychology, social work, or a related field, as per TAC rule §.
- Training in working with individuals with Severe and Persistent Mental Illness and Children with Severe Emotional Disturbances, and delivery of services based on curriculum.
- Strong understanding and ability to work with diverse populations, including children and families.
- Therapeutic, communication, and organizational skills, with intermediate proficiency in Microsoft Office Products.
- A valid driver's license.
