We are seeking a highly skilled and compassionate Care Coach RN Case Manager to join our team at UnitedHealth Group Inc. As a key member of our interdisciplinary team, you will play a vital role in coordinating care and providing ongoing nursing assessment of patients and their families.
Key Responsibilities- Conduct medical home visits on established patients, performing hands-on exams and assessments to identify patient needs and develop individualized care plans.
- Coordinate care with interdisciplinary team members, including physicians, social workers, and other healthcare professionals, to ensure seamless transitions and optimal patient outcomes.
- Provide ongoing assessments of patients' physical, functional, psychosocial, and environmental needs, and develop strategies to address these needs and improve patient quality of life.
- Assess caregiver burnout and provide community resources and support to patients and their families as needed.
- Implement individualized care plans and recommend revisions as necessary to ensure optimal patient outcomes.
- Perform procedures, including wound care, phlebotomy, and injections, as needed.
- Conduct advanced care discussions with patients and their families, and document their wishes in the electronic medical record.
- Initiate preventive and rehabilitative nursing procedures as needed, and ensure ongoing evaluation of patient and family responses to care.
- Collaborate with the interdisciplinary team to develop and implement care plans, and participate in regular team meetings to discuss patient progress and address any concerns.
- Provide telephonic nursing follow-up and case management as needed, and serve as a resource and consultant for other team members.
- Establish trusting relationships with patients, caregivers, and other healthcare professionals, and provide excellent customer service to ensure patient satisfaction and loyalty.
- Registered Nurse with a current license to practice in the state of employment.
- Current BLS certification or must obtain certification within 30 days of employment.
- 3+ years of experience in a physician's office, clinical, hospice, or hospital setting.
- Proficient computer skills, including Microsoft Word, Excel, Access, and Outlook.
- Demonstrate excellent nursing skills, including assessment, planning, implementation, and evaluation.
- Demonstrate ability to interact productively with individuals and multidisciplinary teams.
- Have reliable transportation for daily travel to various locations as assigned, up to 80% of the time.
- Have a valid driver's license within the state of work.
- Hospice and Palliative Care Nursing certification.
- 2+ years of hospice experience.
- Experience with home visits.
- Knowledge of palliative and hospice medicine.
- Experience related to advanced care planning and discussions with patients regarding end-of-life wishes.
- Experience with community-based nursing.
- Bilingual (English/Spanish) language proficiency.
- Excellent verbal and written skills.
- Excellent organizational and prioritization skills.
- Ability to lift up to 100 pounds.
- Ability to push or pull heavy objects using up to pounds of force.
- Ability to sit for extended periods of time.
- Ability to stand for extended periods of time.
- Ability to use fine motor skills to operate office equipment and/or machinery.
- Ability to properly drive and operate a company vehicle.
- Ability to receive and comprehend instructions verbally and/or in writing.
- Ability to use logical reasoning for simple and complex problem solving.
