Job Summary

We are seeking a highly skilled and compassionate Care Coach RN Case Manager to join our team at UnitedHealth Group Inc. As a key member of our interdisciplinary team, you will play a vital role in coordinating care and providing ongoing nursing assessment of patients and their families.

Key Responsibilities
  • Conduct medical home visits on established patients, performing hands-on exams and assessments to identify patient needs and develop individualized care plans.
  • Coordinate care with interdisciplinary team members, including physicians, social workers, and other healthcare professionals, to ensure seamless transitions and optimal patient outcomes.
  • Provide ongoing assessments of patients' physical, functional, psychosocial, and environmental needs, and develop strategies to address these needs and improve patient quality of life.
  • Assess caregiver burnout and provide community resources and support to patients and their families as needed.
  • Implement individualized care plans and recommend revisions as necessary to ensure optimal patient outcomes.
  • Perform procedures, including wound care, phlebotomy, and injections, as needed.
  • Conduct advanced care discussions with patients and their families, and document their wishes in the electronic medical record.
  • Initiate preventive and rehabilitative nursing procedures as needed, and ensure ongoing evaluation of patient and family responses to care.
  • Collaborate with the interdisciplinary team to develop and implement care plans, and participate in regular team meetings to discuss patient progress and address any concerns.
  • Provide telephonic nursing follow-up and case management as needed, and serve as a resource and consultant for other team members.
  • Establish trusting relationships with patients, caregivers, and other healthcare professionals, and provide excellent customer service to ensure patient satisfaction and loyalty.
Requirements
  • Registered Nurse with a current license to practice in the state of employment.
  • Current BLS certification or must obtain certification within 30 days of employment.
  • 3+ years of experience in a physician's office, clinical, hospice, or hospital setting.
  • Proficient computer skills, including Microsoft Word, Excel, Access, and Outlook.
  • Demonstrate excellent nursing skills, including assessment, planning, implementation, and evaluation.
  • Demonstrate ability to interact productively with individuals and multidisciplinary teams.
  • Have reliable transportation for daily travel to various locations as assigned, up to 80% of the time.
  • Have a valid driver's license within the state of work.
Preferred Qualifications
  • Hospice and Palliative Care Nursing certification.
  • 2+ years of hospice experience.
  • Experience with home visits.
  • Knowledge of palliative and hospice medicine.
  • Experience related to advanced care planning and discussions with patients regarding end-of-life wishes.
  • Experience with community-based nursing.
  • Bilingual (English/Spanish) language proficiency.
  • Excellent verbal and written skills.
  • Excellent organizational and prioritization skills.
Physical and Mental Requirements
  • Ability to lift up to 100 pounds.
  • Ability to push or pull heavy objects using up to pounds of force.
  • Ability to sit for extended periods of time.
  • Ability to stand for extended periods of time.
  • Ability to use fine motor skills to operate office equipment and/or machinery.
  • Ability to properly drive and operate a company vehicle.
  • Ability to receive and comprehend instructions verbally and/or in writing.
  • Ability to use logical reasoning for simple and complex problem solving.
